All participants must first have an assessment administered to identify strengths, weaknesses,
problematic areas, leisure interests, and other
additional information needed to facilitate effective
treatment and services

A Treatment Plan is designed. The
Treatment Plan includes specific goals
and objectives that addresses the
participant’s problematic areas.

Together we develop a schedule that
identifies the duration, frequency, and nature
of the participant’s experiences throughout
sessions. Resources, goals, objectives, and
preferences influence schedule development.

Consistent monthly progress notes are
completed and communicated at
participant’s request.

Periodically, therapist and client
will administer evaluations to
determine the progress of the
individual and the program.
Within live through rec treatment
Assessment
Live Through Rec. begins the therapeutic
recreation process by administering an
assessment on all our participants.
During the assessment phase baseline
information is gathered on our participants
cognitive, social, physical, emotional, and
leisure functioning to determine his or hers
interests, expectations, strengths, limitations,
program needs, and current status in each
area.
This information provides the foundation from which we will design a treatment plan in order to reach independent functioning and fulfill full quality of life

Goal and Objectives
Goals are general statements that identify
intervention priorities and describe our expected
outcomes.
SAMPLE: Increase cognitive/academic functioning. To enable participant to learn how to read a book on grade level. For each goal, Objectives are written specifying measurable client outcomes. Objectives are specific, measurable, and time phased so there is no doubt on what outcome to expect and when to see them accomplished. SAMPLE: At the conclusion of the therapeutic session, participant will be able to read at least 50 words per minute with 70% retention rate within 3 months.

Evaluations
Evaluations are done periodically. The
evaluation process summarizes our
participants progress by assessing their
physical, cognitive, emotional, and social
functional abilities.

Interventions
Live Through Rec. therapists use leadership
skills, specialized intervention techniques, and
unique personal skills to establish a strong
therapeutic relationship. Our therapists also adjust client participation by modifying activities in order for each individual to receive appropriate experiences conducive to their plan of treatment.
